August 7, 2017 The Health Care Transformation Task Force (HCTTF) recently announced that ConcertoHealth joined its membership. ConcertoHealth is one of more than 40 members that make up the task force, which is a consortium of leading healthcare payers, providers, purchasers and patient organizations working to transform the U.S. health care delivery system.
Primary care physician registration is now underway for Patient3D, ConcertoHealth’s new proprietary population health data analytics platform. The tool give PCPs a comprehensive, 360-degree view of patients’ complete medical history and consolidated records in near real-time, at the point of care.
